Community Courts Case Manager

Specialty Court - Paw Paw, MI - Full Time

Pay Rate: $24.81-33.80 based on experience

  Full-Time 37.5 hrs per week

JOB SUMMARY:  This position is focused on providing comprehensive outreach, education, and resource connections to individuals struggling with substance use disorders, mental health challenges, and co-occurring conditions. The Community Court Case Manager will work closely with enrolled participants, taking a highly personalized approach to identifying their unique needs and then locating and facilitating access to the appropriate health, social, and community-based services. This involves a deep level of assessment and care coordination to ensure participants receive the holistic support they require. Additionally, the case manager will be responsible for identifying workforce development opportunities and enrolling participants in basic training and employment services. This role is centered around a participant-focused service delivery model, with the case manager serving as a dedicated advocate and guide to help individuals navigate complex systems and access the resources necessary for recovery, stability, and overall wellbeing. Through this multifaceted approach, the case manager plays a vital part in empowering participants, connecting them to vital support, and fostering long-term positive outcomes in the areas of mental health, substance use, and self-sufficiency.


ABILITIES, KNOWLEDGE, EXPERIENCE AND SKILLS:
  • Must possess knowledge of evidence-based substance abuse treatment, mental health treatment, child welfare services, and correctional (probation/parole) strategies to be effective.
  • Must possess excellent oral and written communication skills, advanced organizational skills, and exceptional interpersonal and problem-solving skills.
  • Must possess basic computer skills including applications such as Excel and Sharepoint.

PRINCIPLE DUTIES AND RESPONSIBILITIES:
The following duties are normal for this position.  These are not to be construed as exclusive or all-inclusive.  Other duties may be required and assigned.
  1. Interview and screen treatment court referrals and make recommendation regarding program enrollment.
  2. Administer trauma screening, risk/needs assessment and other instruments as needed.
  3. Evaluate assessment results and prepare written and oral reports for community court team members.
  4. Cooperate with the Treatment Liaison and community court team in staffing to identify optimum treatment placement for participants.
  5. Identify problems in all other life domains to be addressed while in program.
  6. Build relationships with community partners based on identified gaps in services.
  7. Acts as a liaison with appropriate ancillary service agencies such as vocational, rehabilitation, education, transportation, legal aid, substance abuse, and other treatment services.
  8. Identify the full range of service needs for effective case planning.
  9. Orients and guides clients through the program, policy, procedures, and treatment processes
  10. Collaborate with providers (as needed) to address barriers or challenges related to assessment, service coordination or planning, and/or crisis intervention.
  11. Meet with participants on a regular basis to monitor progress and track compliance with court- ordered requirements.
  12. Maintain confidentiality while working with sensitive information.
  13. Enter all screenings into the case management system, tracking enrollment outcomes and client demographics.
  14. Transport participants to long-term and local resource agencies when no other options are available.
  15. Monitor participants’ sobriety by conducting observed urine testing when needed.
  16. Coordinate steering committee meetings.
  17. Attend statewide and national conferences.
  18. Perform other tasks as assigned.


QUALIFICATIONS / EXPERIENCE:
High School Diploma or GED required. Degree preferred. Possession of a valid Michigan driver’s license with acceptable driving record. Experience working in substance abuse, mental health, probation, child welfare, or other court setting preferred. Those with lived experience are encouraged to apply

OTHER REQUIREMENTS:
The individual in this position is an at-will employee.  Must adhere to the Court’s Model Code of Conduct for Judicial Branch Employees.  Employment is dependent on background check, criminal history and fingerprint check, and drug screen.  Applicant must complete County application form.

WORK LOCATION AND PHYSICAL DEMANDS:
Requires the ability to operate a variety of office equipment such as computer terminal, typewriter, telephone, fax machine, calculator / adding machine, printer and copier. Tasks involve the ability to exert light physical effort in sedentary to light work, but which may involve some lifting, carrying, pushing and pulling of objects weighing in excess of 25 pounds. Tasks may involve extended periods of time at a keyboard or workstation.
This position works in an office setting, but some work is conducted in an unsecured environment using sound judgment regarding the supervision of defendants. Work can be performed in the community, jail facility and in the offender’s place of employment. There is automobile travel required to conduct home visits, attend meetings, and transport participants. Work may take place on evenings, weekends and holidays as directed.
